How might we facilitate the discharge experience for outpatients & family members?

MyCare Kit is the primary tool used to guide and support the full member journey. Will be received pre-admission, brought along to inpatient stay, and throughout recovery.

During this project, I sorted through post discharge documentation from Author and outside sources to develop 2 key journeys & touch points in the discharge process.

From there, Remo & I were able to join in zoom calls with Author by Humana members to understand the users' needs & form the design criteria / concepts.
